Distinguished Awards Nomination Form

Please complete a separate form for each nomination. You may submit the form multiple times to nominate additional candidates.
Carefully review the award criteria to ensure your candidate is eligible for the proposed award.

Each nomination must include the following enclosures: (incomplete nominations will not be considered)

  • Reasons for Nomination
  • Candidate CV or Resume
  • Two letters of support from AO members

Describe why you believe this individual or group meets the criteria for the selected category. Your statement should be 200-300 words and detail their achievement and contributions to implantology and/or the AO.

Please note: Nominations will not be considered until CV/resume and accompanying letters of support must be received by August 15 of each year to be considered for that year's Awards cycle.
